I am a 30 year old Cystic Fibrosis patient who has CFRD (CF related diabetes), chronic sinusitis, and a hard time balancing all I need to do with CF, working and being a wife. I created this foldable guide that has a check off list for all the health things I need to do in a day. I also added a section where I could put weekly items to keep track of and where I could tally up my weekly achievements and some diabetes things that aren't set to hard core dates.
It not only helps me on a daily basis remember everything I need to do, but it helps me improve myself and not stress about everything I need to do. I also have used these to bring to clinic to brain storm with clinic when I'm feeling like I need a new/revised treatment plan.
